This appendix lists all 19 records that reached manual review. No exclusion is hidden behind a summary count. The title, reporter, date, source, decision, and rationale come from the same evidence manifest used to calculate the article’s results.
- Included product defect
- Confirmed faulty product behavior counted once in the defect cohort.
- Excluded
- A reviewed report that did not meet the product-defect population rule.
- Deduplicated
- A pull request used as evidence for an included issue, not counted as a second report.
- Process incident
- An externally visible workflow or provenance failure analyzed outside product-defect percentages.
